Instructions for filling out forms:
IMPORTANT: You must print out forms #1 – 3 below and bring them with you to your first team meeting. If you have any questions, please contact the Athletics Department at (617)879-2305.

4. General Amateurism and Eligibility Form for International and Select Student-Athletes – Division III **

* If you are under 18 years old you will need a parent or guardian's signature on this form.

** If you are an international student looking to participate in intercollegiate athletics, please fill out the General Amateurism and Eligibility Form for International and select Student-Athletes. As an intercollegiate athletics participant it is vital that you check your health insurance status to be certain your family policy provides for international coverage .

If you are looking to transfer to another institution, you will need to fill out the Permission to Contact: Self Release Form and submit that form to the institution you are looking to transfer to.
